Consent form for the use of Photographs and Videos of parents and children

In an effort to recognize student achievement and highlight SchoolNova programs, we often take photographs at school events for use in print, in online media publications and on the SchoolNova website. Local news media might also be invited to cover events. Sharing school news helps the community stay informed with the latest happenings at SchoolNova and it is a tribute to the accomplishments of our students, our school and our community. We hope to include your child in coverage of school-related programs, projects and events.
We would like to ask for your consent to take photographs/videos of those events or activities that may contain images of your child.

As a Parent/Guardian,

I give permission for my child(ren) to participate in media covered school events;

I give SchoolNova and it’s official staff my permission to take photos or otherwise capture the image of my child(ren) for any legal use, including but limited to: publicity, copyright purposes, illustration, school advertising and web content;

I grant SchoolNova the right to edit, use and reuse said products for purposes mentioned above. I also hereby release SchoolNova and its staff from all claims, demands and liabilities whatsoever in connection with the above.

We understand if you choose not to consent to photographing your child(ren). If you explicitly do not wish to grant us such permission, please fill out and sign the attached form and return it to SchoolNova Director to director@schoolnova.org.
